New Patient Registration
Registration & Appointment Booking
New to our clinic? Follow these steps to register and book your first appointment:
- Visit our portal and select “Patient” at Priority Health Clinic
- Select “No” when asked if you have registered before
- Enter “New Patient Visit” as the primary reason
- Choose “In-Person or Phone Visit” (all new patient appointments must be in person)
- Select “New Patient” as the appointment type
- Choose a practitioner (select “Okay” to view others if unavailable)
- Select an available date and time
Important Notes
- Each appointment is for one person. Book separately for family members.
- Please select the same provider if you want the same family doctor.
- If you already have a family doctor at our clinic, you cannot switch providers.

Appointment with Your Family Doctor
Book using our web-based system or the TELUS CHR Connect App. If you can’t find a suitable time, contact the clinic using the methods on this page.
How to Book Online
- Visit: priorityhealth.inputhealth.com/ebooking
- Select Yes if you are already registered
- Log in using:
- Last name
- Date of birth
- Gender
- Ontario Health Card Number (10 digits only; do not include version code)
- Email or phone number
- Enter the one-time passcode
- Select appointment reason
- Choose In-Person or Phone Visit
- Select appointment subtype
- Pick a date and time
- Verify/add demographic information
- Review and submit request
- Receive confirmation and reminders via email or text
Using TELUS CHR Connect
Important: If you already use TELUS CHR Connect for another clinic, log in at chrconnect.telushealth.com. Once logged in, select Add Clinic and search for Priority Health Clinic.

Vaccine clinics are for individuals who have received a school suspension notice due to missing vaccinations.

- Enter “Vaccine Immunization” as the reason for your appointment
- Choose “In Person” (these appointments must be in person)
- Select “Vaccine Clinic” as your appointment type
- Pick a date and time from the available options
How to Book (Registered / Urgent Care Patients)
- Visit: priorityhealth.inputhealth.com/ebooking
- Select “Yes” if registered at our clinic
- Log in using:
- Last name
- Date of birth
- Gender
- Ontario Health Card Number (10 digits only)
- Email address
- Enter the one-time passcode sent to your email
- Choose “Vaccine Immunization” as the primary reason
- Choose “In Person” (these appointments must be in person)
- Select “Vaccine Clinic”
- Pick a date and time
- Verify/update demographic information if needed
- Review and submit request
- Receive confirmation and reminders by email or text
Important Notes
Please note: While Hepatitis B and HPV vaccines are within our clinic’s scope of practice, they will not be provided or ordered during the school-aged vaccine clinic day. Due to the high volume of patients scheduled, the clinic is structured to focus on timely and efficient delivery of routine school-aged immunizations.
Ordering additional vaccines on the day of the clinic requires additional assessment, documentation, and follow-up, which cannot be safely accommodated within the clinic schedule.
If your child requires Hepatitis B or HPV vaccination, we encourage you to contact your family physician to arrange an appointment, either in person or by phone. Your physician can complete the necessary review and place the vaccine order at that time, ensuring appropriate documentation and continuity of care.
- Each appointment is for one person. Repeat booking for additional family members.
- Routine childhood vaccines (such as well-baby visits) should be booked under “Appointment with Your Family Doctor.”
- Please bring your Ontario Health Card.
- Please bring your Public Health immunization form if available.
- If you don’t have the form, you can log in to Durham.ca/immunize to view outstanding vaccines before your appointment (recommended to do in advance).
- If forms are not available, the appointment may still proceed with documentation of required vaccines.
